Before you proceed, please choose the relevant option based on your medical institution's HCSA licensing status and ensure you have the following information ready. Please note that clinics applying for the Community Health Assist Scheme ("CHAS") would need to be accredited under the MediSave and MediShield Life Scheme for Chronic Disease Management Programme (CDMP).

Please also note that the Public Health Preparedness Clinic (“PHPC”) Scheme is only applicable for GP clinics.
