COMMUNITY HEALTH ASSIST SCHEME OVERVIEW

(A) Overview of Community Health Assist Scheme


1.      The aim of the Community Health Assist Scheme (CHAS), formerly known as the Primary Care Partnership Scheme (PCPS), is to provide needy elderly and/or disabled Singaporeans with better access to primary healthcare by bringing affordable services closer to such residents. Under the CHAS, the Ministry of Health (MOH) engages private general practitioners (GPs) and dentists to provide common outpatient medical and dental treatment to the needy elderly and/or disabled at subsidised charges.




2.      Only Health Assist cardholders, Community Medical Benefit (CMB) cardholders or Public Assistance (PA) scheme cardholders are eligible for treatment under the CHAS.

3.      Today, the scheme covers acute primary care conditions, chronic conditions, and dental procedures.

4.      The treatment of chronic diseases under the CHAS is aligned with the Chronic Disease Management Programme (CDMP) framework and the chronic diseases that will be included in this extension are Diabetes Mellitus, Hypertension, Lipid disorders, Stroke, Asthma, COPD, Schizophrenia, Major depression, Dementia, and Bipolar Disorder.

(B) Application


5.      To be eligible for government subvention under the CHAS, the medical clinic as well as the GP providing medical treatment must be accredited under the Medisave for CDMP framework. Please click here for more information on the Medisave/Medishield Accreditation Scheme.

6.      After the medical clinic has been accredited under the Medisave for CDMP framework, it will be invited to sign a new CHAS agreement with the polyclinic cluster assigned.

Accreditation and Registration for Medical Clinics

7.      Clinics that are currently not yet CDMP-approved can jointly register for accreditation under the Medisave for CDMP framework and sign-up for CHAS here. In the selection menu that appears, please indicate that you would like to participate in the Medisave/Medishield scheme and proceed as instructed.

8.      For clinics that are currently CDMP-approved and who wish to sign-up for CHAS only, please click here. In the selection menu that appears, please indicate that your clinic is currently CDMP accredited and proceed as instructed.

9.      Once the clinic's CDMP-accreditation has been successfully approved, the assigned polyclinic cluster will contact the clinic to sign the CHAS agreement.
