This application is only relevant to medical practitioners who wish to make MediSave and MediShield Life claims for patients via an accredited medical institution. Please seek the agreement of the medical institution for clinical privileges at their premises before proceeding with this application. Please note that GPs who wish to administer CHAS will need to have clinical privileges at a CDMP-accredited clinic.

This accreditation is valid for only 2 years. After expiry, you may re-apply at this website.

Before you proceed, please ensure that you have the following information ready:

1. Personal details of the medical practitioner
2. MCR/DCR number
3. Email address of the medical practitioner
4. Agreement of the MediSave and MediShield Life accredited clinic where the medical practitioner is granted clinical privileges.
